What’s the most gruesome/tragic/heroic Last Breath failure you’ve seen in your games?

Not looking for deals with death, I mean 6- results.

  1. I had a guy who was fighting a monstrous bloated mummy full of corpse worms. As he attempted to grapple with it, the corpse worms poured out from its flesh into him, tearing him to pieces. Probably one of the more gruesome deaths I’ve done
